Discharge planners often give families just a few days to decide. It is a lot to take in while you are also worried about your parent or spouse. You do not have to figure it out alone, or pay out of pocket because you ran out of time. This is what we do every week.
Discharge usually goes one of three ways

Home with funded care
For when home is safe with the right support. We help set up funded home care through Community Medicaid, including CDPAP, where an approved family member can be paid to provide the care.
A nursing home
For when the level of care is too high for home. We help with Nursing Home Medicaid and the asset protection planning that goes with it, so the cost does not wipe out the family.
