    What is Community Medicaid?

    Community Medicaid is the part of New York's Medicaid program that pays for long-term care in your own home. It can cover home health aides, personal care, and the CDPAP program, where a family member can be the paid caregiver.

    The goal is simple. It lets your loved one stay in the home they are comfortable in, with help coming to them. This is different from Nursing Home Medicaid, which covers care in a facility and follows stricter rules.

    Who qualifies for Medicaid home care in New York?

    Eligibility comes down to three things:

    • New York residency.
    • A medical need for help with daily activities like bathing, dressing, walking, or managing medication.
    • Meeting Medicaid's income and asset guidelines.

    That last point is where families count themselves out too early. New York's asset rules are far more generous than most states, and there are legal tools, like a pooled income trust, that can help people with higher income still qualify. Before you assume you make too much or have too much, it is worth getting real answers.

    A planning window you should know about

    For nursing home care, New York reviews the past five years of your finances before approving benefits. Community Medicaid for home care has historically had no such look-back, which gives families more flexibility.

    As of 2026, that is still the case. New York has passed a 30-month look-back for home care, but it has not been put into effect yet and could be activated in the future. That makes planning now especially valuable, while the more flexible rules are still in place.
